Cap Number, Age & Date of Birth. Also, a check for your Championship Fees.

that is fine. Using this sheet is not mandatory, but all content
represented here must be on your roster. Also, players should be listed in
cap number order.)
NOTE:
last year some coaches did not have complete rosters and I let it go. This
year that will not be the case.
IF YOU DO NOT HAVE COMPLETE ROSTERS AT THE MEETING, YOU WILL FORFEIT THE
